BASIC PURPOSE:
The Renal Dietitian (RD) is responsible for the nutritional assessment, intervention, education and ongoing nutritional evaluation of the hemodialysis or peritoneal dialysis patients on her/his caseload. The RD interacts with all health care team members in the application, education, and communication of nutrition care principles.
Patient Care:
The Renal Dietitian (RD) is responsible for the nutritional assessment, intervention, education and ongoing nutritional evaluation of the hemodialysis or peritoneal dialysis patients on her/his caseload. The RD interacts with all health care team members in the application, education, and communication of nutrition care principles.
Patient Care:
- Completes initial comprehensive nutrition assessment and plan of care for assigned patients, including intensive nutritional counseling s needed. Documents assessment and intervention in the patient database. Maintains essential records for each patient.
- Complete nutrition assessments for case load on a quarterly basis or more often as medical/nutritional needs dictate.
- Performs monthly lab reviews as related to diet and nutritional status and documents.
- Educates patients and/or care givers, including extended care facilities, on principles and practice of the individually prescribed renal diet, accounting for patient food preferences.
- Is routinely available to assigned shift of patients for questions and/or counseling.
- Participates in patient care conferences including documentation on the short and long term care plans.
- Designs, evaluates and updates patient education materials. Contributes to the patient newsletter as assigned.
- Prioritize daily schedule to complete most important patient interactions/tasks first.
- Communicates effectively and professionally with all members of the patient care team.
- Plans ahead, receives approval of the Nutrition Services Coordinator, and posts work schedule, allowing for optimal patient care and communication among patient care staff.
- Attends mandatory in-service education programs. Makes arrangements with the Facility Manager if such an in-service is missed to illness or changes in the routine work schedule.
- Participates in Quality Outcome projects and department audits.
- Complies with infection control policies.
- Effectively communicates with patients, referring doctors, hospitals, and extended care facilities regarding optimal nutrition care.
- Promotes community and public relations. Maintains involvement with professional organizations. e.g. Kidney Foundation of Ohio, Northern and Eastern Council on Renal Nutrition, Cleveland Dietetic Association, or the American Dietetic Association.
- Supervises dietetic students/interns participating from area colleges or universities in clinical or outpatient affiliations
- Initiates and/or participates in research related to nutrition and renal disease.
- Complies with the personnel policies of the organization.
- Demonstrates dependable work performance and attendance
- Exhibits appropriate behavior when staff and/or patient grievances occur.
- Exhibits professionalism in all staff and patient interactions
- Adjusts work schedule during absence of other department members if necessary.
- Maintains Oho license and registration status.
- Attends and participates in continuing education activities pertinent to clinical experience that will improve the quality of patient care including Journal Club
- Registered, licensed Dietitian having completed the American Dietetic Association approved program for registration eligibility.
- Minimum of one year experience in clinical nutrition, preferably renal experience.
- Evidence of continuing education in dietetics to maintain registration status.
